Your Health Matters Programme

Your Health Matters’ is a 16-week behavioural change programme targeted at people experiencing issues linked to diabetes, smoking cessation, high BMI and wanting to improve general health and wellbeing.

During the structured programme you will receive regular communication and support from our friendly experts helping with recipe ideas, weekly activity goals, lifestyle changes and mindset goals.

‘Your Health Matters’ consists of elements that can be completed in person, online and in the comfort of your own home alongside a workbook.

You will be supported, and your progress will be monitored throughout the programme to ensure your personal goals are achieved.  When you are nearing the end of the programme we will discuss a variety of ‘exit route’ opportunities, ensuring you continue progressing on your health and wellbeing path and sustained behavioural change.

What to expect on the programme

  • Monthly in-person group sessions that will be themed around different topics e.g. physical activity, healthy eating, importance of hydration etc.
